List Site Instances

GET 
/session/sites/:site_id/instances

Get all instances for a specific site that the user has access to.

Site ADMIN users (role='ADMIN' in users table) see ALL instances in the site. Other users see only instances they have explicit permission to access. Uses new RBAC system (user_instance_roles and users.role).
